Theory: Theory
is a set of concepts or ideas or assumptions accumulated together to explain
the reality and usually follow a set of principles. For example:-

Model:
Simply, model is an abstraction of reality. More specifically model is a
logical & mathematical linkage of concepts, ideas and assumptions. Model is
more concrete form of theory. Model includes some interdependent &
interrelated ideas or logics that generates output. For example:-
