The
Japanese quality expert Ishikawa formulated a diagram which is helpful in
identifying and analyzing the cause for a particular problem existing in an
organization. Causes in the diagram are often categorized, such as to the 6
M's, 7P’s, 5S’s. Causes can be derived from brainstorming sessions. These
groups can then be labeled as categories of the fishbone. Causes can be traced
back to root causes with the 5 why (Where, What, When, Who, Why) technique.

(technology), Method (process), Material (Includes Raw Material, Consumables
and Information), Man Power (physical work)/Mind Power (brain work), Measurement
(Inspection), Milieu/Mother Nature (Environment)
The
7 Ps (used in marketing industry)
Product=Service,
Price, Place, Promotion, People/personnel, Process, Physical Evidence
The
5 Ss (used in service industry)
Samples, Scheme, Synchronous, Skin, Search
Firstly the
major cause of problem are placed or sketched in the diagram
Then the sub
causes are generated according to category
Among the sub
causes the actual causes are concentrated
for eliminating them
Finding out
solution by solving the actual cause of the problem
